By booking, entering, or receiving services from Leviathan Ink or Hit’em Records, you agree to the following Terms & Conditions in accordance with Indiana state law.

1. General Agreement

All clients must read and agree to these Terms & Conditions prior to receiving services. Failure to comply may result in refusal of service without refund.

Studios reserve the right to amend these terms at any time.

2. Indiana Age & Identification Requirements

  • A valid government-issued photo ID is required for all services.

  • Indiana law prohibits tattooing of minors under 18 years of age — NO exceptions.

  • Body piercing of minors is only permitted when allowed by Indiana law and requires documented parental consent and parent/guardian presence.

  • Fake or altered IDs will result in immediate refusal of service.

3. Health & Safety Disclosure (Indiana Code Compliance)

Clients receiving tattoo or piercing services must disclose:

  • Medical conditions (including bleeding disorders, diabetes, epilepsy, immune conditions)

  • Allergies (latex, metals, inks, disinfectants)

  • Medications that affect healing or blood clotting

  • Skin conditions or infections

Failure to disclose required medical information releases Leviathan Ink from liability under Indiana assumption-of-risk principles.

4. Appointments & Deposits

  • Appointments may require a non-refundable deposit.

  • Deposits apply toward the final service total unless otherwise stated.

  • Deposits are forfeited if cancellation or rescheduling policies are not followed.

5. Cancellations, Rescheduling & No-Shows

  • Minimum 3 hours’ notice is required for cancellations or reschedules.

  • A 15-minute grace period applies.

  • Late arrivals, no-shows, or failure to give proper notice may result in:

    • Loss of deposit

    • Additional fees

    • Future booking restrictions

6. Payments & Pricing

  • All prices are subject to artist, piercer, or engineer discretion.

  • Pricing may vary due to time, complexity, design changes, or material costs.

  • Accepted payment methods may include cash and electronic payment options.

  • Tips are appreciated but not required.

7. Tattoo & Piercing Aftercare & Liability (Indiana Law)

  • Aftercare instructions are provided verbally and/or in writing.

  • Leviathan Ink is not responsible for healing issues caused by:

    • Failure to follow aftercare

    • Improper cleaning or exposure

    • Pre-existing conditions

  • Touch-ups are not guaranteed and may involve additional costs.

8. Recording Studio Policies (Hit’em Records)

  • Clients must arrive on time and prepared.

  • Studio time includes setup, recording, playback, and cleanup.

  • Session files are the client’s responsibility after delivery.

  • Hit’em Records is not liable for lost data after the stated retention period.

  • Unauthorized recording, duplication, or distribution inside the studio is prohibited.

9. Conduct & Right to Refuse Service

Under Indiana business rights and safety laws:

  • Disruptive, threatening, harassing, or intoxicated behavior will not be tolerated.

  • Studios reserve the right to refuse or terminate service for any reason related to safety, legality, or professionalism.

  • Removal due to misconduct forfeits all payments and deposits.

10. Refund Policy

  • No refunds for completed services.

  • Deposits are non-refundable.

  • Any exceptions are at management’s sole discretion and handled case-by-case.

11. Assumption of Risk & Waiver of Liability

Clients acknowledge that tattooing, piercing, and professional recording environments involve inherent risks. By receiving services, clients voluntarily assume all risks and waive liability to the fullest extent permitted under Indiana law.

12. Media, Surveillance & Promotion

  • Video and audio recording may be used for security.

  • Promotional media may be captured unless declined in writing prior to service.

13. Governing Law & Venue

These Terms & Conditions are governed by the laws of the State of Indiana, and any disputes shall be resolved exclusively in Indiana courts.

By booking or receiving services, you confirm that you have read, understood, and agreed to these Terms & Conditions.
